The shift key could be depressed and you were originally meant to rest your pinky on it, and lock (same key) for all uppercase / extended punctuation.
It's a total throwback to very old technology.

Functional requirements documents are mundane, drool documents that can make you die a little inside. http://www.fivesimplesteps.com/products/a-practical-guide-to-managing-web-projects is a fantastic book which tries to lay out a few alternatives.

There are lots of good individual articles here: https://longform.org/ some are templates, some are "art directed" but they stay current.
