I do all my organization digitally using Evernote.

Being a student, I take pen/paper notes during my classes, then scan them or photograph them with my smartphone straight into Evernote. From there I can put them into a relevant notebook and add tags. All the writing is searchable and I can also search for notes I took on a specific date.

On reality I do notes on a lot of paper, scramble paper. But evernote definitely help me to organize my notes. Especially there are converter/ scanner from photo to notes directly to your evernote. Amazing

I generally take my notes by iPad, sometimes on paper, but I've moved between a few tools for keeping them digitally and ended up settling on... Apple Notes (after Evernote and Simplenote with various clients, including nvAlt).

Notes does 90% of what I need: * simple formatting * great sync (99% of the time) * fast

As for structure, I keep one note per project, and add new stuff to the top, from the proposal outline to requirements to interviews to development and release notes. Sync and search have remained very quick despite such big documents, which kind of surprised me.

I do miss not being able to include PDFs and other media, but having simple, fast, and synced notes outweighs that by a lot.

For paper goods, I love MUJI and often use their products: clean, functional, beautiful.

You might be able to find something that fits your needs on their website: http://www.muji.us/store/stationery/folders-binders.html

For digital goods, I love Simplenote — free, does 1 thing but super well: keep text notes synced across devices. http://simplenote.com

    hope this helps!

+1 for Muji stuff. They have a store in Manhattan and their paper products are quality and cost-effective. Much cheaper than a "name brand" notebook like Moleskin.

I generally keep a table of contents in my notebooks. Each page with new content gets a page header and a corresponding entry (plus a date) at the beginning of the notebook.
