Tank Design http://www.tankdesign.com/ in Boston. They specialize in branding and identity but also provide other services. I think their client list speaks for itself...

Here's my hit list (in order of cost):

Pentagram NYC - Expensive, and picky, but in my humble opinion still the best brand folks in the business.

Huge Inc. SF - The SF office is focused on digital products, and lead the rebrand of Fitbit.

Meta Design SF - These folks did some brand work for my former employer. 4-years later, the work they did still translates well to multiple clients and platforms. http://www.metadesign.com/clients/inkling

We used SoftFacade for the LayerVault app icon and glyph. They are awesome.

I would suggest Handsome (http://handsome.is) in Austin, especially if you're looking for any design work in addition to brand/identity. They're very web/mobile focused.
