Nice work! Really liking the visuals as well. It's nice to see something different than those boring old and seemingly huge forms you have to fill out. Must say I was a bit disappointed to see that there was no natural language input, which the dotted line kind of made me expect. Since the input format is static [Date] [Brand] [Make], you can provide suggestions based on what the user is typing in at that moment?
Believe it or not, we used to have natural input, and it was often cited as a pain point (go figure). When I designed the madlib, I was planning on using typed entry, but the feedback swayed me towards a picker.
Small upside is the picker works pretty dang smooth on mobile devices. You can just tap tap tap with your thumb and be done in seconds.
If you want to see remnants of the natural input, go into the app view and visit the vehicle tab, where the live-search input is still in place.
